Minor Participant Waiver & Release of Liability
As the parent or legal guardian of the minor named below, I acknowledge and agree to the following:
- Assumption of Risk
I understand that Ecstatic Dance involves physical movement, which carries inherent risks, including but not limited to slips, falls, collisions, and physical exertion. I voluntarily assume full responsibility for any risks, injuries, or damages associated with my child’s participation. - Medical Authorization
In the event of a medical emergency, I authorize the event organizers and staff to seek medical attention for my child if I am not present. I acknowledge that I am responsible for any medical expenses incurred. - Supervision & Responsibility
I understand that I am responsible for supervising my child at all times during the event. The event organizers, facilitators, and venue staff are not responsible for lost children, accidents, or injuries due to lack of supervision. - Release of Liability
I, on behalf of my child, myself, and any representatives, waive, release, and discharge Ecstatic Dance Charleston, its organizers, facilitators, venue partners, and all related personnel from any and all claims, liabilities, or damages arising from my child’s participation in this event. - Photo & Video Release
I grant permission for photos or videos taken during the event that include my child to be used for promotional purposes.
